Enclosed you will find the Nebraska High School Rodeo Application, National
High School Rodeo Application and the Nebraska State Entry Form. Each member
needs to fill out these forms for the 2011-2012 rodeo season. Please fill them
out and return them to Becky Dailey, Box 10, Arnold, NE 69120.
Go to hsrodeo-nebraska.com, under Schedules and print off a Fall Schedule.
There will also be information on rodeos in the Fence Post.
Here is a checklist for each form. MAKE SURE THE FORMS ARE COMPLETELY
FILLED OUT CORRECTLY. If they are not I have to return them to you, which
makes it longer before, you will get your membership packet. So please fill them
out completely. Do not forget to put your Social Security Number on the
National Form; the National office requires your SS#.
2011-2012 Membership Application
- Include a check for your membership of $175.00.
- Include your last semester’s grade report. If you are a renewing
member and submitted grades for state finals, we have your grades on
file and you won’t need to submit your grades with this application.
- Make note of the calendar fundraiser-deadlines and fines. If you
are a new member, joining after March 1st, you are only required to send
in $200. for calendars. If there are calendars available, arrangements
will be made to get them to you.
NATIONAL HIGH SCHOOL RODEO APPLICATION
- Fill out both sides
- Mark what events you want to be eligible to compete in. If you
are thinking about competing in an event, make sure you mark it. This
saves from having to mail the form back to you for signatures.
- Have BOTH PARENTS & MEMBER sign the form, front and back
- If you come from a single parent family, the single parent needs
to list divorced with full custody or deceased. Parents with joint
custody must include both signatures even if the child will be living
with only one parent during the rodeo season.
- On back put SOCIAL SECURITY NUMBER. I must have this.

- Must be Notarized
2011-2012 Nebraska High School Rodeo Form
- BOTH parents must sign next to each event that you want to
compete in at the qualifying rodeos. If you are thinking about competing
in an event, check the event and have your parents sign off on the
events. This saves having to mail the form back to you for signatures.
- Must be Notarized
- Both Parents and member or legal guardian must sign at bottom of
form
I cannot express to you how important it is to fill out the above forms
correctly. It saves time and work for me if they are done right the first time.
If they are not filled out completely they will be returned to you which will
delay your membership packet.
